Q: Where are your offices located?
A: We are in
the Rusk Rehabilitation Building at 400 East 34th Street, located
between First Avenue and the FDR Drive. You will need a photo ID to
enter the Rusk building. Once inside Rusk, please take the K elevator
to the 3rd Floor.Turn right off of the K Elevator and our offices are
located in the suite at the end of the hall.
Q: What should I bring for my first visit?
A: Please bring a letter of referral from your doctor that includes
your medical history.We will also need any lab or test results that you
may have as well as a detailed medication list that includes dosages
and daily schedule.
Q: How much time should I allow for my appointments?
A: We conduct a thorough evaluation for all of our new patients, so
please allow a minimum of 2 hours for your first appointments and up to
an hour for follow-up visits.
